JPB BELIEVES IN...

Below, are the program forms (JPB Covenant Agreement, JPB Risk/Release Agreement, JPB Media Release) required for each participant to fill out and hand-sign (for minors, parent/guardian signature required). Please write out your responses clearly.  Once finished, email high-resolution images or scans of the forms to peace@jerusalempeacebuilders.org. Program forms that are unreadable will need to be resubmitted. The deadline to submit your program forms is May 1, 2022.
